Advanced Directive
It is the policy of Johnson Memorial Health to allow all adult patients with decision-making capacity (including emancipated minors) to participate in decision-making concerning their health care and medical treatment. Advance directives shall be followed by Johnson Memorial Hospital to the extent permitted and required by Indiana law.
Medical Treatment Authorization
This grants temporary authority to a designated adult to provide and arrange for medical care for a minor in the event of an emergency, where the minor is not accompanied by either parents or legal guardians, and it may not be feasible or practical to contact them.

Everyone has a role in making health care safe - physicians, health care executives, nurses and technicians. Health care organizations across the country are working to make health care safety a priority. You, as the patient, can also play a vital role in making your care safe by becoming an active, involved and informed member of your health care team.

Personal Items
Upon admission to the hospital, patients will be instructed to send valuables home. If you are unable to send items home, Johnson Memorial Hospital will provide secure storage for items. The hospital assumes no responsibility for replacing items that are not placed in the secure area for safekeeping.
What To Bring
- Insurance information and valid picture identification.
- A list of medications you are currently taking (prescription and non-prescription).
- An Advance Directive (a living will and/or durable power of attorney for health care) if you have one.
- Personal care items such as toothbrush, toothpaste, comb, shaving products, feminine hygiene products, shampoo and soap.
- Things to do while resting: a book, crossword puzzles, etc.
- Charger(s) for cell phones and other electronic devices.
