Direct Access Lab Testing
JMH offers a selection of affordable laboratory tests designed to evaluate your current health status with no physician order required. Payment is due at the time of service. Master Card, Visa, Discovery cards are accepted; your insurance cannot be billed.
Please complete the Direct Access Test Charge Form and present it to the cashier for payment.
Your Results
Access your tests results through MyJMH Patient Portal, direct mail, contacting your physician or at Johnson Memorial Hospital. You must present a picture ID and sign a Request and Authorization for Disclosure of Medical Record Information form to pick up your results. No results will be released via telephone or fax.
